ROOTED is designed exclusively for Medicaid recipients in Columbus, Ohio. If your young man meets the criteria below, we'd love to hear from you.
Eligibility criteria
- Boys ages 14 through 16
- Active Medicaid coverage
- Residing in or near Columbus, Ohio
- Open to participating in mentorship and therapy sessions
What it costs
ROOTED is Medicaid-approved. There is no out-of-pocket cost for eligible families. Our team handles the coordination so you can focus on your young man's growth.
How to enroll
Step 01
Reach out
Send us a message with your young man's name, age, and Medicaid status — or have a caseworker contact us on his behalf.
Step 02
Intake conversation
We meet with you and your young man to understand his goals, needs, and what kind of support will help him most.
Step 03
Welcome & onboarding
Once enrolled, he's matched with a mentor and begins his behavioral-health and mentorship sessions in a structured weekly rhythm.
